In consideration of my acceptance of participation in the P.F. Chang's Rock 'n' Roll Arizona Marathon & Half Marathon as part of Team Tillman, I hereby for myself, my heirs and administrators waive any and all rights and claims for damages I may have against the Pat Tillman Foundation and will hold them harmless for any and all injuries suffered in connection with said event. Also, the Foundation is not responsible for the loss of personal items or any other form of aggravation in connection with said event. I have been warned I must be in good health to participate in this event. In filling out this form, I also give permission for free use of my name and picture in any broadcast and print media accounting this event. I understand that by setting up my event page on this site, I am making a commitment to raise a minimum of $500 in donations and that my credit card will serve as a guarantee. I understand my credit card will be charged for any balance not raised through my fundraising efforts after the deadline of January 2, 2009. In filling out this form, I acknowledge that I have read and fully understand my own liability and accept the restrictions.
